DAY HABILITATION SURVEY
We are exploring an expansion our CONNECT Day Habilitation program into evening events for persons served by the state's Habilitation Plan.
A survey is being distributed to consumers, family members, case workers and other stakeholders to gather interest.
The CONNECT program currently serves persons M-F during day hours with activities on site and in the community.
Expansion would allow people not currently served to experience greater involvement in their community.
Any interest or ideas are welcome!
